Periodic Alternating Nystagmus Associated with Visual Loss

Abstract
An 80-year-old man with bilateral blindness due to macular degeneration showed periodic alternating nystagmus (PAN) changing direction every 30-90 seconds. Other neurological examinations and brain MRI were unremarkable. Our case suggests that the deficient visual input is crucial for the generating PAN and the putative mechanism is instability of the velocity-storage mechanism due to loss of visual inputs to the nodulus and/or uvula.
